Transition Program

The MUSC CF Center believes in age-appropriate care.  We begin preparing the patient and family for transition into adulthood from the time of our first meeting.  By the time patients are 18 years of age, they are ready for transfer to the adult team.  We work towards the development of the patient’s independence, enhancing their knowledge of CF, and their acceptance of ownership of their care.
