The Assignment Process

Incident Report

The following is the recommended procedure to be followed if a student is involved in any incident that has potential professional liability or legal implications:

  • The student should immediately notify the CI of the incident and follow the facilities procedures regarding reporting.
  • The student should notify the ACCE of the incident as soon as possible, following completion of the facilities procedures. The ACCE should be provided with a written copy of the incident report.
  • Upon receipt of any official document notifying the student of any compensable event, the student must furnish a copy of the report to the ACCE.
  • The ACCE will furnish a written copy of the report to the Division of Physical Therapy Division Director, the Department of Health Professions Chair, the Medical University of South Carolina attorney and the professional liability carrier.
