To Request a Cumulative Report of CME Credits, please follow the applicable steps below.
Please allow a 48-hour turnaround time to receive your report.
If you do not receive yours within 48 hours, please call The Office of CME at 843-876-1925.

Profession (MD, DO, NP, RN, etc)
Your MUSC Employee ID

   (This ID number can be found at https://perlapp.musc.edu/mydirectory/. This link can only
     be accessed by a MUSC computer. If you are unable to access the link, please leave this blank.)

Your Department

Time Period Requested (Example: July 1, 2011 to June 30, 2013)

Optional: Fax or Mailing Address. If you'd like your report mailed or faxed, rather than emailed, please indicate which you prefer.


If you are not an MUSC Employee:
You may obtain your report for a $25 fee. Please call our office at 843-876-1925 with credit card information available.

Please Note: If you are requesting a report including credits obtained more than 2 years prior to the date of request, there will be a $30 fee. Please call our office if you have any questions, 843-876-1925.
