Educational Record Release

The link below provides students with a means of granting permission to an instructor, advisor, or school official to write a recommendation letter to include specific information about the student's GPA, class rank, or other parts of their educational record in compliance with the Family Educational Rights and Privacy Act (FERPA).

For convenience, the form itself is not required, and the student may instead copy and paste its text into an email to the instructor, filling in the information regarding recipient(s).

Furthermore, this consent form, is not required by FERPA if the instructor only intends to include personal impressions about the student regarding aptitude or abilities, without reference to GPA, class rank, or other non-directory elements of the student's official educational record.
