Submission Process

Submissions are accepted via email, and signature pages may be scanned and sent via email:

New Protocol Applications | Existing Protocols

New Protocol Applications

  1. Complete Vertebrate Animal Review Application (VARA).
  2. Complete all applicable appendices.
  3. Sign Principal Investigator Statement of Assurance (VARA, signature page).
  4. Obtain Department Chair's signature (VARA, signature page).
  5. Obtain applicable corollary approvals (IBC, Radiation Safety, Risk Management).
  6. Submit all materials via email, and signed pages may be scanned and sent via email.
  7. Respond promptly to any requests for clarification/revision.
  8. Approval will be emailed to PI after appropriate review is completed. This requires that all comments/concerns are adequately addressed, necessary signatures are on file, and all applicable supporting materials are submitted.

Existing Protocols

Continuing Review | Change in Personnel/Change in PI
Amendment to Protocol | 3-Year Renewal

Continuing Review

  1. Required at the end of years 1 & 2 after Initial or 3-Year Renewal.
  2. Requested prior to the approval anniversary.
  3. The completed and signed form may be scanned and sent via email.
  4. Reviewed prior to expiration by the full IACUC, usually at a meeting.
  5. Approval of the Continuing Review extends the protocol for one year.
  6. Approval notification will be emailed to PI after review.

Note: Submission and/or approval of the Continuing Review does not substitute for submission of an amendment if changes to the approved protocol are needed.

Change in Personnel/Change in PI

  1. Required to add or remove personnel, or to modify duties (i.e. staff member who was approved for only euthanasia will be performing surgery). Required to change Principal Investigator of study.
  2. All sections for the applicable change must be completed.
  3. The completed and signed form may be scanned and sent via email or faxed.
  4. Change in Personnel requests are approved on an expedited basis .
  5. Approval notification will be emailed to PI.

Amendment to Protocol

  1. Required to make any changes to previously approved protocol, except for Change in Personnel/Change in PI.
  2. All applicable changes must be indicated. If changes are restricted to those indicated in the first section of the amendment form, a revised narrative will likely not be requested as long as changes are adequately described/justified in the area provided at the bottom of the page. Changes in species or methods/procedures must be accompanied by a revised narrative with all changes highlighted for reference.
  3. Explanation/justification should fit in space provided at bottom of page. This portion should be brief and specific. Details should be included in the revised narrative.
  4. The completed and signed form may be submitted via email, along with any other documention.
  5. Respond promptly to any requests for clarification/revision.
  6. Approval will be emailed to PI after appropriate review is completed. This requires that all comments/concerns are adequately addressed, necessary signatures are on file, and all applicable supporting materials are submitted.

3-Year Renewal

  1. This is a de novo review as mandated by federal law.
  2. Please follow the New Protocol Application directions.
  3. Training protocols and protocols including USDA-covered species are not eligible for 3-Year Renewal. Protocol requests beyond the 3-year approval period must be submitted as a new protocol.
