CAMR User Guide

CAMR = Chart of Accounts Maintenance Request
UDAK = User Defined Accounting Key

The UDAK is the key for the University's financial records in SmartStream.

Each MUSC department establishes and maintains UDAKs, following University Financial Policy, to organize the department's business and financial records as part of the financial records of MUSC. 
 
The CAMR form provides the way for a department to maintain UDAK information.  There are different formats based on the type of UDAK needed.  These are explained in the Finance Policies and Procedures for Accounting (linked from the left menu).

CAMR FORMS:                  

The following Acrobat PDF versions of the CAMR have default information coded.
Do not change this default information.  
These PDF forms are not "fill-in" ready.  You must print or save them locally. 
Once the form is complete, you can send to the Controller's Office, MSC 817 or email as an attachment to finrept@musc.edu.
