Long term care refers to a broad range of medical, personal and social services provided to people who are unable to care for themselves over an extended period of time. Services include nursing home care, adult day care, assisted living facilities or home and community based care. You select the amount you would like reimbursed for these services based on a daily benefit amount. The premium is made payable directly to Prudential. The criteria to begin receiving benefits is the inability to perform two of six daily living activities (bathing, dressing, eating, toileting, transferring or continence) or a severe cognitive impairment that requires supervision. Once approved by Prudential, benefits are payable after a once in lifetime 90-day waiting period. Participants eligible for coverage after approval of medical evidence of good health, include: -all actively-at-work, full-time permanent employees; -relatives of the eligible employee including spouses, parents, parents-in-law, grandparents, grandparents-in-law, siblings, adult children age 18 and older and their spouses; -retirees and their spouses; and -surviving spouses. All new hires who are actively-at-work, benefits eligible employees in a permanent position may enroll within 31 days of their hire date and are guaranteed coverage without medical evidence.
